    • Select Client:  type in the name of the client if this is an individual you have seen for prior Engagements or in other Customer Programs.  Where , commence typing in the client’s name and where relevant, the client’s details will then populate relevant fields

    • First and Last Name:  this is a mandatory field and MUST be completed in full as this function allows the linking of prior/future case notes for this individual.  All clinicians are required to obtain accurate first and last name details of each client. Be mindful of entering correct spelling.

    • Email

    • Country code:  automatically populates with AU/AUS code

    • Phone number:  this is mandatory field and is important as this function allows the linking of prior/future case notes for this individual.  All clinicians are required to obtain accurate phone number details for each client.

    • Date of Birth

    • Employee ID: where relevant

    • Session Date:  type the date or use the calendar icon

    • Session Duration:  select the duration of the one-on-one session from the drop-down list

    • Description:  provide a concise description of the group session

    Client Checklist

    • Has the privacy statement been provided:  choose Yes/No

    • Has the introductory checklist been completed:  choose Yes/No

    • Has consent been provided:  choose Yes/No

    Case Notes

    • What issue did the client present with:

    o   Issue Category:  select the issue category from the drop-down list

    o   Issue:  select the issue from the drop-down list

    • Was a mental health risk assessment completed:  choose Yes/No

    • Describe the presenting issues and any other case notes 

    • What were the types of interventions, techniques and strategies utilised?

    • Did you refer the client to EAP or any other services:  choose ‘Referred to EAP’ / Referred to Other / No Referral

     

    Note

    It is mandatory for all consultants to obtain full first and last names of each client, and their preferred contact number. It is not suitable for fields such as ‘N/A', 'X', or other symbols to be used.

    1. Select ‘Complete’ when all notes are completed, or ‘save & close’ to allow a draft to be saved where
