First and Last Name: this is a mandatory field and MUST be completed in full as this function allows the linking of prior/future case notes for this individual. All clinicians are required to obtain accurate first and last name details of each client. Be mindful of entering correct spelling.

Country code: automatically populates with AU/AUS code

Date of Birth
Employee ID: where relevant
Session Date: type the date or use the calendar icon

It is mandatory for all consultants to obtain full first and last names of each client, and their preferred contact number. It is not suitable for fields such as ‘N/A', 'X', or other symbols to be used. |

There is a cancel option also if you wish the user wishes to cancel this entry.
