Case Note Requests - Employees
Consultants are expected to maintain adequate case notes for every EAP Client. It is important that session notes reflect the parameters of our service. Only that which is client reported should be noted alongside strategies provided, homework and any Consultant/Client follow up agreed upon. Please do not use diagnostic language or provide any commentary that may be viewed as professional opinion. There is a high frequency of requests for case notes in the EAP space from clients, third parties and subpoena requests which is managed by privacy@convergeintl.com.au.

As an employee, you are contractually obligated to use our case note system on Phoenix. Please refer to Viewing & adding case notes (via Quick View)
Reviewing case notes on Phoenix
If your case notes are on Phoenix, the Privacy team will request that you review them before they are ready for release. The client details will be provided in an email for you to review and confirm they can be released to the client. Please refer to Viewing & adding case notes (via Quick View)
Reviewing archived case notes
If Converge has a copy of your case notes as they are located on X Drive or archived (prior to uploading on Phoenix), the Privacy team will request that you review them before they are ready for release. Converge International uses a file sharing platform called ShareFile. It is imperative that we use a secure file sharing system when sending and receiving highly sensitive and confidential information, pertaining to customer case notes. If we have a copy of it, we will send it on the ShareFile link in the email.
