Completing a request form
If you need to complete a request form, such as Counselling services provided to a minor or Release of information consent form, our Customer Service team will provide you a link to download the PDF form.
When completing the form, you can choose to either:
Download, then complete the form on your device, digitally and email the form to Converge International to the email indicated on the form
OR Download and print the form, complete it by hand, then scan and email the form to Converge International, to the email indicated on the form
How to digitally complete the form on your device
Follow the instructions for the device type you are digitally completing the form on:
If you have a free Adobe Account:
Download the form from the web-page provided to you by Converge International Customer Services
Go to the Online Adobe Fill & Sign tool https://www.adobe.com/acrobat/online/sign-pdf.html
Tap or Click “Select a file”
Select the file from your downloads folder or from the location on your device where you saved the form
Sign into your Adobe account or create a free one
After reviewing the contents of the form, complete the form and sign
Download your completed form
Finally, create a new email to the email address specified in the form and attach your completed form
If you are downloading on an iPhone or iPad
Using Safari:
On the web-page provided by Converge, tap the Download Form button
You’ll now see the form; save the form by tapping the Share icon
On the share screen, scroll down and tap Save to Files then tap Save
The downloaded form will be in your Files Application ; Open the Files app and go to Recents from the bottom menu
Once you find the the downloaded PDF, tap to open it
After reviewing the form, scroll to the section where you need to enter your data
Tap on a section where you need to enter data and the keyboard will appear
Complete all sections where you need to type information, then tap Done when complete
To sign you signature where required, tap the Pencil icon
Use two fingers to zoom in to the section, then use your finger to sign
Tap Done when complete
The completed form will save in the same place in the Files App (downloads folder)
You can now re-open the completed form and then tap the share icon to send it by email.
Using Google Chrome (on iPhone and iPad):
On the web-page provided by Converge, tap the Download Form button
A pop-up will appear with the PDF and a link that says “Open in…” - tap this
Scroll down and tap Save to files then tap Save
The downloaded form will be in your Files Application
Open the Files app and go to Recents from the bottom menu
Once you find the the downloaded PDF, tap to open it
After reviewing the form, scroll to the section where you need to enter your data
Tap on a section where you need to enter data and the keyboard will appear
Complete all sections where you need to type information, then tap Done when complete
To sign you signature where required, tap the Pencil icon

If you are downloading on an Windows PC or Mac
Using Microsoft Edge (Recommended) (on PC or Mac)
On the web-page provided by Converge, tap the Download Form button
Once downloaded, click the three dots at the top right
From the menu, click “Downloads”
Click the PDF form from the list of downloads
You’ll now see the PDF form; review the contents of the form
Once you’ve finished reviewing the form, scroll to the section where you need to enter your data
Click and enter text as needed
Once completed, enter your signature by clicking on the Draw at the top left
Then, using your mouse, click and draw the signature in the sign section
Then, click the Save As icon and save to desktop or where you prefer
Finally, create a new email to the email address specified in the form and attach your completed form
Using Google Chrome (on PC)
Note, Google Chrome supports downloading and viewing forms and entering data but does not support signing the form. You’ll need to download Adobe Acrobat and then follow the steps below
You’ll need to download and instal Adobe Acrobat: get.adobe.com/reader/
On the web-page provided by Converge, tap the Download Form button
Next, find the downloaded file on your computer and open it with Adobe Acrobat
In Adobe acrobat, review the contents of the form
Once you have reviewed the form, scroll down to the section where you need to enter data
Click on the sections to enter text
Once completed entering all text, you’ll need to sign the document
Click the “All Tools” icon at the top left
From the list of “All tools”, click “Fill & Sign”
Sign the section asking for a signature using your mouse by clicking and dragging your mouse to draw your signature
Save the form
Finally, create a new email to the email address specified in the form and attach your completed form
