If you would like to request your case notes, you will need to complete a Release of Information form. This can be accessed here: http://www.convergeinternational.com.au/clientforms/ricf

Please note that the access to and provision of case note files can take up to 28 days (or longer depending on the age of the file) from the date that the completed and signed Release of Information form is sent to the Privacy team.

Accessing your case notes

Converge International uses a file sharing platform called ShareFile. It is imperative that we use a secure file sharing system when sending and receiving highly sensitive and confidential information, pertaining to customer case notes. Once we have obtained your cases notes and have confirmed they are ready for release, we will send it on the ShareFile link in the email.

Step 1: Click on the link

  1. To continue, you will need to fill out the below details.

    1. Email

    2. First Name

    3. Last Name

    4. Company

  2. Press Continue
